RB7 Futsal Contract 

 

A. Purpose of Contract 

RB7 Futsal is a program built for players that want to take advantage of supplemental training for outdoor soccer as well as being involved in a professional, competitive futsal environment throughout the year. Contracted players will be expected to attend one session per week. 

 

B. Fees 

3 Month Contract/ 1 session per week (December 1, 2024 – February 28, 2024)

3 Month Contract/ 2 sessions per week (December 1, 2024 – February 28, 2025)

_____ $470 Paid in Full Fee

 

Payments should be submitted using one of the following methods:

C. Terms of Contract

  • All new players will pay a $30 Administration Fee when they register and sign a contract.

    • Any player who stops training with RB7 Futsal for any period of time or not continually under contract will be required to pay the Administration Fee when returning and signing a new contract. 

    • Players remaining under contract will pay a yearly Administration Fee to RB7 Futsal due on March 1. 

  • All players will purchase a practice jersey from RB7 Soccer and Futsal Academy.

    • Cost of practice kit is $35.

    • Returning players are not required to purchase a new jersey unless needed.

    • The RB7 practice jersey will be worn to weekly practice sessions.

  • Players will train once a week at the designated monthly practice session for the fees indicated above. 

  • Fees must be paid to RB7 Futsal no later than the 5th day of every month. After the 5th day, an additional $10 late fee will be charged.

  • Any additional practice session(s) attended by a contracted player will be at the cost of $25 per session paid to RB7 Futsal. 

  • Players may have an option to attend training sessions two times per week. That option requires approval from the RB7 Futsal Staff.

 

D. Length of Contract 

3 Month contract term--December 1, 2024 – February 28, 2025.

 

E. RB7 Futsal Loyalty to Players

RB7 Futsal will give contracted players the opportunity to renew their contract for three-month periods. All players must sign a contract to train and play under RB7 Futsal. If the contracted player does not wish to renew his or her contract, the RB7 Futsal staff has the right to replace that player with another who is willing to fulfill the standards of this contract.

 

F. Number of Groups

Depending on the needs and skill level of each player, as well as the demand for players for each age group, RB7 Futsal has the right to divide or combine players into particular training groups. These training groups will be organized to benefit each individual player’s developmental needs. These training groups may consist of players within the same age or of varying ages. The selection for training groups is at the full discretion of the RB7 Futsal Staff.

 

G. Waitlist for Players

If the number of players reaches capacity for an age group, players that want to attend RB7 Futsal sessions will be put on a waitlist until a spot becomes available or there is a sufficient number of players for an additional group and available court time. Once there is availability, the parent/guardian must sign the RB7 Futsal Contract as well as pay their registration fee and first month training fee prior to attending RB7 training sessions. 

 

H. Team Selection for Tournaments

RB7 Futsal Players may be asked to compete as part of a tournament team, however, no player is guaranteed a spot on a tournament team. To explain further, although certain age groups may have 10 or more players, it is possible and likely that the RB7 Futsal Staff only selects 7 players per age group to attend a tournament. The selection for tournament teams is at the full discretion of the RB7 Futsal staff. 

 

I. RB7 Futsal Guidelines 

RB7 Futsal Players are expected to:

-    Attend every session.

-    Treat coaches, teammates, and other players with respect at all times.

-    Pay attention and listen to all coaches.

-    Be focused and coachable. 

-    Be honorable to the facility, its staff members, and management. 

 

 

J. Termination of Contract

Although not expected, RB7 Futsal may terminate a contract if:

- A player does not abide by the RB7 Futsal Guidelines

- The player misses three or more sessions without notifying the RB7 Futsal Staff.

- RB7 Futsal does not receive the adequate amount of payment from player’s parent or guardian.

 

K. Payment Requirement

The parent/guardian must fulfill their obligation to pay all fees for the contracted training period regardless of a player’s attendance at training sessions. If the contract is terminated by RB7 Futsal Staff for any of the reasons mentioned in Section J., the parent/guardian must fulfill their obligations to pay the remaining contract balance to RB7 Futsal.

 

L. Freedom to Train 

This contract in no way deters players from taking advantage of other training sessions. As long as the player fulfills their end of the RB7 Futsal contract, the player can attend sessions elsewhere as much as they desire. 

 

M. Media Release

The RB7 website and social media accounts may be used to share photos and videos of RB7 players during trainings, games, and tournaments. By signing this contract, the parent/guardian grants permission for their child’s image to be used for these purposes. If a parent/guardian wishes to opt out of this release, please do so in writing and send to rb7academy@gmail.com.

 


MEDICAL TREATMENT AUTHORIZATION,

INFORMED CONSENT AND ASSUMPTION OF RISK AGREEMENT


(FOR PARTICIPANTS 17 YEARS OLD AND YOUNGER)

WARNING! By signing this document you will waive certain legal rights, including the right to sue in circumstances outlined
in this Agreement. Please read carefully.

1.            This is a binding legal agreement; therefore clarify any questions or concerns before signing. As a Participant participating in the sports of futsal and/or soccer, including training, competitions and practices (collectively the “Activities”), the undersigned, being the Participant and the Parent/Guardian of the Participant (collectively the “Parties”) acknowledge and agree to the following terms:

2.            RB 7, LLC and its coaches, managers, volunteers and representatives (collectively the “Organization”) are not responsible for any injury, personal injury, damage, property damage, expense, loss of income or loss of any kind suffered by a Participant during, or as a result of, the Activities, or caused by the risks, dangers and hazards associated with the Activities.

 

Description of Risks

3.            The Participant is participating voluntarily in the Activities. In consideration of that participation, the Parties hereby acknowledge that they are aware of the risks, dangers and hazards and may be exposed to such risks, dangers and hazards. The risks, dangers and hazards include, but are not limited to, injuries from:

a)                The sports of soccer and futsal;

b)                The hazards particular to the activity(ies) in which the Participant is participating;

c)                Executing strenuous and demanding physical techniques;

d)                Vigorous physical exertion, strenuous cardiovascular workouts and rapid movements;

e)                Exerting and stretching various muscle groups;

f)                 Grass, turf and other surfaces, including infections and rashes;

g)                Falls to the ground due to uneven or irregular terrain or surfaces;

h)                Collisions with other participants, walls, stands and futsal/soccer equipment;

i)                  Extreme weather conditions which may result in heatstroke, sunstroke or hypothermia;

j)                  Dry land training including weights, running, bands and circuit;

k)                Failure to properly use any piece of equipment or from the mechanical failure of any piece of equipment;

l)                  Contact, colliding, falling or being struck by other participants or equipment that may cause traumatic brain injury and/or other injuries or death;

m)               Spinal cord injuries which may render Participant permanently paralyzed;

n)                Travel to and from Events which are an integral part of the Organization's Events.

4.            Furthermore, the Parties are aware:

a)                That injuries sustained can be severe;

b)                That the Participant may come into physical contact with other participants;

c)                That the Participant’s risk of injury is reduced if the Participant follows all rules established for participation; and

d)                That the Participant’s risk of injury increases as the Participant becomes fatigued.

Release of Liability

5.            In consideration of the Organizations allowing the Participant to participate, the Parties agree:

a)                That the Participant’s physical condition has been cleared by a medical doctor to participate;

b)                To freely accept and fully assume all such risks, dangers and hazards and possibility of personal injury, death, property damage, expense and related loss, including loss of income, resulting from the Activities;

c)                To forever release, waive, hold harmless and indemnify the Organization from any and all liability for any and all claims, demands, actions and costs that might arise out of the Participant’s participation in the Activities, or from any breach of contract.

 

Medical Clearance and Medical Treatment Authorization

6.            I hereby give my consent, on my own behalf or on behalf of my child or guardian, as applicable, to have an athletic trainer, coach, team manager, emergency medical technician, physician, nurse, dentist, or other healthcare professional, and in each case their associated personnel, provide the player identified below with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and treatment.  I understand treatment for injury will be based, at least in part, on information provided herein.  I hereby authorize emergency transportation of the player, at player or parent’s/guardian’s expense, to a healthcare facility should an individual listed above consider it to be warranted.  In signing below, I certify that the player received all necessary medical clearances to participate fully in all programs provided by the Organization without restriction or condition.

Acknowledgement

7.            The Parties acknowledge that they have read this agreement and understand it, that they have executed this agreement voluntarily, and that this Agreement is to be binding upon themselves, their heirs, executors, administrators and representatives.
